Traditionally, clinicians learned how to practice medicine and to choose interventions by using various approaches--consulting experienced colleagues, searching through textbooks, listening to expert panels, and heeding their own experiences. However, no physician has a sufficiently large practice to make epidemiologically sound judgments, and our feelings about treatments tend to be colored by recent, and particularly by bad, experiences. Thus, the science of clinical investigation has grown up, and many have begun to advocate that clinicians make choices about therapy and diagnosis and make recommendations for prognosis based on the available evidence.

This concept is referred to as evidence-based medicine (Evidence-Based Medicine Working Group, 1992). When using an evidence-based approach, practitioners make decisions by reviewing the available evidence about a given therapy or situation instead of simply asking a colleague about it. This demands that a set of skills be developed so one can learn how to evaluate different types of studies and become facile at performing literature searches. Fortunately, there are now guides about how to do this effectively. This approach can be applied to all areas of medicine, not only gynecology. However, many within gynecology have called for the profession to make this change (Frigoletto, 1997; Olive and Pritts, 1997; Peipert and Bracken, 1997; Zinberg, 1997).
