Interventional cardiology has moved from coronary angioplasty to percutaneous interventions on the heart and vasculature. This issue of Cardiology Clinics focuses on coronary artery interventions. We are fortunate to have Dr. Samin Sharma, who directs one of the premier cardiac interventional laboratories in the United States, as guest editor for this issue. He and his colleagues have contributed a great deal to the advancement of the technical aspects of coronary interventions. They and other experts from the United States have put together an outstanding comprehensive discussion of current issues in coronary interventions, including a discussion of who should have a percutaneous coronary intervention and safety concerns.

The appropriate application of any technology is often its Achilles' heel. Being accomplished at doing something does not necessarily translate to having good judgment about when to use this skill. Ideally good judgment is developed over time; hence the development of interventional cardiology training programs, boards, and the special certificate in this subdiscipline of cardiology. With coronary artery bypass surgery providing very similar outcomes, patient selection in more challenging patients is crucial. Also, advances in the management of cardiovascular disease and heightened expectations of patients are bringing many 80- and 90-year-olds who have advanced vascular disease to the catheterization laboratory. Although bypass surgery seems overly aggressive and is often not welcomed in such patients, one must be able to comprehend the limits of percutaneous interventions also. Many difficult situations confront the interventional cardiologist today, and this issue of Cardiology Clinics provides an excellent database for approaching management decisions in patients who have coronary artery disease.
