We know it has helped many readers to successfully overcome examinations, but, most rewarding of all, is the recognition of the effect on patient care derived from the clinical usefulness of our text. An appreciation for this impact carries with it a profound sense of obligation. We have always understood the importance of getting it left and causing no harm. For that reason, every edition is the culmination of a tremendous effort, an effort increasingly difficult with the amazing growth of knowledge and communications. In 1972, it was a Royal portable typewriter. At the millennium, we are armed with a personal computer and a modem, Word 97 for the text, EndNote for references, and MEDLINE for searches,

The book grows progressively thicker, reflecting the explosion of knowledge in reproductive biology and medicine. Nevertheless, we believe we have stayed true to our objective: to provide both factual knowledge and systematic approaches to clinical problems. Our text continues to be an expression and formulation of our collective professional activities as teachers, investigators, and most importantly, as clinicians in the field of reproductive endocrinology and infertility. We have tried to retain the style and clinical relevance of our first edition, but at the same time to provide all the new information. This basic knowledge is the foundation for the physiologic principles that in turn are the foundation for our methods of clinical management.

As always, our book is dedicated to the improvement of patient care, and we hope that it will aid all readers in accomplishing that goal. We are grateful to those who have taught us, and most of all, we thank our patients, who are our best teachers.
